Appointment Procedure
All family members who are applying to be enrolled in WIC must come to the first WIC appointment. You will need to bring the following information for each family member who is applying for WIC:
- Proof of identification for participant(s) and parent, guardian or proxy:
- Birth certificate, hospital record, or crib card
- Photo ID (school ID, state ID, drivers license, passport, military ID)
- WIC ID
- Proof of current entire household income:
- Child support check stub
- Current pay or unemployment check stub
- Current Notice of Decision Letter (Food Share or Medicaid)
- Forward Health Card
- Proof of current residence:
- Any document with your name and current address
- Child / children's immunization records
- Current bill
- Proof of pregnancy
- Rent receipt or lease with address included
